
Grant Self-Assessment Report Process
Trellis Foundation believes that self-reflection and assessment improves ministry effectiveness. So we ask all grant recipients to complete a grant self-assessment after the conclusion of their initiative. Some may be asked to complete a progress report partway through their initiative.
Click the button at right for the online Self-Assessment Report form. Grant recipients may refer to their original grant approval letter to determine when the report is due.
