Trellis Foundation believes that self-reflection and assessment improves ministry effectiveness. So we ask all grant recipients to complete a grant self-evaluation after the conclusion of their initiative. Some will also be asked to complete a progress report mid-way through their initiative.

These documents may downloaded by clicking on the appropriate button to the right. Grant recipients may refer to their original grant approval letter to determine which document(s) are needed and when they are due.
